This is a supreme declaration and the goal of the human form of life. Just like a king does have a palace some where in the capital of a country, the Lord has His own planet in the spiritual universes. And because all the living entities are His parts and parcels, the human form of life is so special that can realize God or can become Krishna Conscious so that the same consciousness will enable the departing soul to be transferred to that spiritual kingdom where the Lord lives. Then the goal of the human form of life will be attained.
But what happens when one cannot think of the Supreme Personality of Godhead Krishna at the time of death? The answer to this lies in the freedom given to the human beings and the consequences they have to bear of their own actions. One will naturally think of the activities he or she has been engaged and the desire thereof to enjoy the fruits of those activities, even at the last moment of life. The answer is thus obvious. Whatever state of being one remembers when he quits his body, O son of Kunti, that state he will attain without fail- Bhagavad Gita 8.6. Here it is the fact to be understood that if some one is thinking of his pet dog at the time of death he/she will get the body of a dog in the next life. That is the result of his consciousness. But if he thinks of the Lord, he will have a spiritual body of eternal bliss and happiness in the spiritual planet. After all, we are spirit souls originally, but because of our forgetfulness of the spirituality, we are covered and conditioned to suffer in different bodies life after life. Therefore to break this cycle of sufferings, the Krishna consciousness process taught by the world’s most empowered spiritual personality Srila Prabhupada is the divine gift to all. There is no other process although many preachers do say that whichever way you may follow, or do always good, you will reach the same destination. The Lord declared in BG.9:25- Yanti deva..that ‘Those who worship the demigods will take birth among the demigods; whose who worships the ancestors go to the ancestors; those who worship ghosts and spirits will take birth among those beings; and those who worship Me will live with Me’. This is where life goes after death.

A Vaisnava or one who is Krishna conscious would help one attain a better life on this earth and the next- after death too. Kevin prabhu, in his discourse elaborated his help to the departing soul, that he chanted the maha mantra and offered the auspicious items that invoke the consciousness towards the divine existence. What could be the result of a successful death can also be visible in the body of the dying one and the environment he is in. Passing stool or urine at the time of death would mean to suggest that he saw terrible things tormenting him and was terribly afraid at the time of leaving the body. This would mean that ghost or spirits and or yamaduta has taken that soul for the final judgment. The conditioned soul identifies him with the body and taken away by his false ego, mind and intelligence he develops a subtle body that undergoes various phases after death.
But a Krishna conscious person do not develop such a false ego, ie identifying the body as the self, and his mind and intelligence engaged in the spiritual or devotional service to the supreme, gets purified and will be able to think of the supreme absolute, the Lord, at the time of death. Thus as declared by the Lord Himself he/she will be able to go to live in the kingdom of God, after death. The death of such a self-realized person will be blessed with devotees around, there will be divine symptoms and death takes as a happy departure, escorted by Vishnu dutas. A very highly elevated pious devotee can even go to the spiritual world without leaving the body behind, and one such example is of Tukaram, a follower of Sri Caitanya Mahaprabhu who is Krishna Himself appearing as a devotee to teach the practical application of the Bhagavd Gita.
